1.     Please provide the necessary diabetic supplies for your child's diabetic care at school.

2.     Diabetes Medical Management Form (DMMP)

• Please make sure that ALL sections of the DMMP are complete, and the form is signed by a physician and parent.

3.     Medication Administration Authorization Form signed by the physician and parent.

4.     Provide emergency medication prescribed.
