2024-2025 Online Pre-Registration Enrollment Form

Please note the above step pre-registers your child. Shortly after submission you will receive a confirmation email. When your student's enrollment is approved you will recieve another emrail with instructions on how to complete the enrollment. 

When completing your online enrollment, you will be required to upload the following documents after the initial pre-registration form is submitted: 

  • Original Birth Certificate with raised seal
  • Copy of Immunization Records
  • Proof of residency (i.e. utility bill, tax statement, house buy/sell agreement, lease agreement)
  • Photo ID of parent or guardian registering student
  • Copy of current IEP if special services placement is required
  • High School Students - copy of latest transcript
  • Health Appraisal - required for Pre-Kindergarten and Kindergarten students only

These items can be submitted in one of the following file formats: DOC, DOCX, PDF, TXT, RTF, XLS, XLSX, BMP, GIF, JPG, JPEG, PNG

If you would rather submit these items in person, please come to our administrative offices located at 3886 Van Buren Street, Hudsonville. We are open week days from 7:30 a.m.-4:30 p.m.
