When enrolling you will need:

• Transcript from current school
• Copy of immunization records Immunization Requirements
• Original birth certificate (with raised seal)
• Proof of Residence
• Photo ID
• Enrollment Form (below)
• Records Release Form (below)
• Student Technology and Equipment Form (below)
• Concussion Form
• Family-School Compact

Records Release Form (for 9th grade) - This form is a fill-in form. Please complete and print. It does not submit electronically.

Student Technology and Equipment Use Policy and Authorization Forms - Please read forms carefully prior to signing.

HPS High School eGuide - Contains information for choosing classes including a pdf version if needed. New students will need to submit course selections on paper as PowerSchool will not be available to them.
